Appointments

We know you have many choices when choosing an Otolaryngologist in Glendale, AZ. If, for any reason, you cannot keep a scheduled appointment or will be delayed, please call as soon as possible. Charges may be incurred for appointments cancelled less than 24 hours before scheduled appointment time.

Insurance

We currently accept Medicare assignment, most commercial plans (HMO's and PPO's), and some AHCCCS (Medicaid) plans. Please check with your insurance carrier to make sure we are in network with your particular plan.  We are contracted with some of the following carriers:
