Beginning January 2020, we converted to "paperless" registration. Three days prior to your appointment, you will receive a text message to confirm your appointment. Once your appointment is confirmed, you will receive another text with a link to register. You can complete this on your cell phone or on a computer or laptop if your e-mail has provided. PLEASE NOTE: If you are unable to complete the registration before your appointment and need to complete it in our office, your check-in time is 30 minutes PRIOR to your appointment.
HELPFUL HINT: If you do have an extensive medication list and/or medical and surgical history, it can be much easier to bring in a list.

To assist with clinic flow, we ask that all new patients complete the registration before your appointment. If you have any other records or testing that may be pertinent to your office visit, please bring those along for the doctor to review.
